随着腔内血管重建手术的不断开展,覆膜支架的应用也越来越多,但是在某些特殊部位使用覆膜支架会影响动脉分支血管的血液供应,如主动脉弓,腹腔干动脉,双侧肾动脉和肠系膜上动脉等。这些特殊部位只能通过传统外科技术或者杂交技术开胸(或者开腹)将临近血管游离,然后进行血管搭桥,暂时保证血运,手术复杂,时间长,且对病人损伤特别大。为了解决上述问题,现有技术中出现了转流管技术,即在覆膜支架阻断重要动脉分支血管之前,先通过转流管对靶器官进行临时供血,然后在一定时间内对覆膜支架进行原位开窗,恢复原来的血液供应,最后撤出转流管。以主动脉弓部夹层累积左锁骨下动脉为例,先在右腹股沟附近切口,暴露右股动脉,然后在加硬导丝的支撑下,通过输送系统经髂总动脉、腹主动脉、胸主动脉、以及降主动脉,将胸覆膜支架送至主动脉弓部近端,锚定区介于左锁骨下动脉与左颈总动脉之间,部分释放胸主覆膜支架,覆膜支架骨架打开,支架覆膜将左锁骨下动脉完全覆盖,左锁骨下动脉血流被阻断,故需要在降主动脉至左锁骨下动脉之间临时建立一条血管通路,维持左锁骨下动脉的血供,待左锁骨下动脉的覆膜开窗完成之后,再撤出转流管,转流管的使用,能使覆膜支架应用到主动脉弓及内脏血管附近的病变,从而避免周围过大手术。现有的文献中Sonesson(2012Newtemporaryinternalintroducershuntforbrainperfusionduringtotalendovasculararchreplacementwithinsitufen ...
